XERF vs. Microneedling vs. RF Microneedling: Which Collagen Treatment Is Right for You?

If you've started researching treatments for firmer, smoother, healthier-looking skin, you've probably encountered a long list of options promising to "stimulate collagen."

Microneedling.

Radiofrequency microneedling.

Radiofrequency skin tightening.

Lasers.

Biostimulators.

And now, XERF.

They may all appear under the broad category of collagen-stimulating treatments, but they aren't interchangeable. They deliver energy differently, target different depths and concerns, involve different amounts of downtime, and may be better suited for different patients.

At Spindrift Aesthetics, I don't believe the goal should be choosing whichever treatment happens to be trending on social media.

The better question is:

What are we trying to accomplish within your skin and which technology makes the most sense for that goal?

Let's break down three popular collagen-focused treatments: traditional microneedling, radiofrequency (RF) microneedling, and XERF.

First: Why Are We Trying to Stimulate Collagen?

Collagen is one of the primary structural proteins within our skin. It contributes to the strength, thickness, firmness, and overall architecture of the dermis.

When we're younger, our skin contains a dense and organized collagen network. With age, collagen production gradually slows while existing collagen becomes increasingly fragmented and degraded.

Sun exposure accelerates that process.

Hormonal changes can influence it.

Smoking, environmental stressors, and other lifestyle factors can contribute as well.

Eventually, we may begin noticing changes such as:

  • Fine lines

  • Crepey texture

  • Skin laxity

  • Enlarged pores

  • Changes in firmness

  • Thinner-looking skin

  • Less definition along the face and jawline

Collagen-stimulating treatments work by creating a controlled stimulus within the tissue. The body responds to that stimulus through a wound-healing or remodeling response that can include fibroblast activity and the production and reorganization of collagen.

The important distinction is how that stimulus is created and where within the tissue it occurs.

That's where microneedling, RF microneedling, and XERF become very different treatments.

What Is Traditional Microneedling?

Microneedling is one of the most established treatments used to improve skin quality.

During treatment, very fine sterile needles create controlled microchannels within the skin. These microscopic injuries initiate the body's natural wound-healing response.

That response includes signaling processes involving fibroblasts and collagen remodeling.

The concept is relatively straightforward:

Controlled injury → healing response → tissue remodeling.

When performed appropriately, microneedling can be particularly useful for improving concerns such as:

  • Skin texture

  • Fine lines

  • Enlarged pores

  • Certain acne scars

  • Overall skin quality

One of the advantages of traditional microneedling is its versatility. Treatment depth can be adjusted depending on the area and indication.

It also doesn't rely on heat.

That distinction becomes important when comparing it with radiofrequency-based treatments.

What Is RF Microneedling?

Radiofrequency microneedling combines two technologies.

First, needles penetrate the skin. Then radiofrequency energy is delivered through those needles into predetermined tissue depths.

Instead of relying exclusively on the mechanical injury created by microneedling, RF microneedling adds thermal energy.

The goal is to create controlled zones of heating within the dermis and, depending on the device and settings, potentially deeper tissues.

This thermal response can produce collagen contraction and stimulate subsequent remodeling.

RF microneedling can be useful when we're trying to address concerns such as:

  • Skin laxity

  • Texture

  • Fine lines

  • Acne scarring

  • Crepey skin

  • Certain areas of deeper tissue remodeling

But adding RF energy also changes the treatment considerably.

It introduces additional variables including needle depth, energy level, pulse duration, tissue impedance, number of passes, and the precise location in which energy is being delivered.

That makes device selection, treatment parameters, anatomy, and provider expertise

What Is XERF?

XERF takes a very different approach.

XERF is a non-invasive radiofrequency technology designed to deliver controlled thermal energy into the skin and underlying tissue without needles.

Rather than physically penetrating the skin to deliver RF energy, the treatment delivers energy externally.

XERF uses dual-frequency radiofrequency technology at 6.78 MHz and 2 MHz, allowing energy to be delivered at different tissue depths.

The controlled heating created during treatment is intended to produce immediate collagen contraction while initiating longer-term collagen remodeling.

For patients, one of the most noticeable differences is the experience.

There are:

No needles.
No numbing.
No open channels in the skin.
And essentially no social downtime for most patients.

That makes XERF particularly interesting for patients who want to support collagen and address laxity but don't necessarily need resurfacing of the skin.

XERF vs. RF Microneedling: What's the Biggest Difference?

Both treatments use radiofrequency energy.

But how that energy reaches the tissue is completely different.

RF microneedling uses needles to penetrate the skin and deliver RF energy at selected depths.

XERF delivers radiofrequency energy externally without penetrating the skin.

That difference affects several aspects of treatment.

Skin Integrity

RF microneedling intentionally creates microscopic channels in the skin.

XERF leaves the epidermal barrier intact.

Downtime

RF microneedling typically involves some degree of redness, swelling, sensitivity, and visible recovery.

XERF generally has minimal to no social downtime.

Treatment Experience

RF microneedling commonly requires topical anesthetic because needles penetrate the skin.

XERF does not require numbing and is designed to maintain patient comfort while the tissue is heated.

Primary Treatment Goal

While there is overlap, I tend to think about these technologies differently.

RF microneedling can be very useful when a patient needs surface remodeling plus thermal collagen stimulation, particularly when texture or scarring is part of the concern.

XERF becomes particularly compelling when the primary goal is deeper structural support, firmness, laxity, and collagen maintenance without disrupting the skin's surface.

Neither automatically makes the other "better."

They solve somewhat different problems.

XERF vs. Traditional Microneedling

This comparison is even more distinct because traditional microneedling doesn't deliver radiofrequency energy at all.

Microneedling relies primarily on controlled mechanical injury. XERF relies on controlled thermal energy.

For someone whose primary concerns are enlarged pores, superficial textural irregularities, or certain acne scars, traditional microneedling may still be an excellent option.

For someone who has beautiful skin quality but is beginning to notice laxity around the lower face, cheeks, or jawline, XERF may make considerably more sense.

Again, we come back to the same principle:

Treat the problem, not the trend.

What About Fat Loss With Radiofrequency?

This is an important conversation because patients increasingly ask about it.

Radiofrequency energy can interact with different tissue depths depending on the device, frequency, energy settings, treatment technique, and anatomy.

Certain RF technologies are intentionally designed to heat subcutaneous tissue and may even be used specifically for body contouring or fat reduction.

That doesn't mean every radiofrequency treatment automatically causes facial fat loss.

But it does mean that energy-based devices should never be treated casually.

Devices such as XERF, lack the ability to cause fat cell destruction. The device and technology behind XERF RF is designed with safety stops which limit the device in temperature deliver—this means the device will not deliver energy when the skin temperature becomes too high. The machine is built with integrative cooling technology meaning that it is not only constantly cooling the skin, but it’s also constantly monitoring the skin temperature during the treatment and prior to the delivery of energy before a pulse is delivered.

The face contains complex superficial and deep fat compartments that contribute to youthful contour and structural support.

As we age, some of those compartments naturally lose volume.

For many patients, indiscriminate destruction of facial fat would be the opposite of what we're trying to accomplish.

This is why I believe strongly in understanding anatomy, understanding the technology being used, and selecting treatment parameters intentionally.

More energy isn't always better. Deeper isn't always better. And aggressive treatment isn't automatically more effective treatment.

Why Preserving Facial Fat Matters

For years, aesthetics focused heavily on reducing or tightening everything.

Now we're developing a more sophisticated understanding of facial aging.

Youthful faces don't simply have tight skin.

They have appropriate:

  • Bone structure

  • Fat distribution

  • Ligament support

  • Muscle balance

  • Collagen density

  • Skin thickness

  • Elasticity

Facial fat plays an important role in that architecture.

This is also why I approach collagen treatments with a long-term perspective.

If we're treating someone in her 30s or 40s who wants to age well over the next several decades, our goal shouldn't simply be creating the most dramatic immediate change possible.

We need to consider what we're preserving for the future.

What Does “Collagen Banking” Actually Mean?

You may have heard me use the term collagen banking. It doesn't mean we're literally putting collagen into a savings account. It's a philosophy.

Instead of waiting until significant laxity and structural deterioration have occurred, we begin supporting the skin's collagen network earlier and maintain it over time.

Think about strength training. You don't wait until you've lost substantial muscle mass to decide muscle is important. Ideally, you build it, maintain it, and protect it throughout adulthood.

I approach collagen similarly.

For some patients, XERF isn't about trying to create a dramatic lift. It's about maintaining strong, healthy tissue and encouraging collagen remodeling before substantial correction becomes necessary.

So, Which Treatment Is Best for Someone in Their 30s?

There isn't one universal answer. A 35-year-old with acne scarring and enlarged pores may benefit more from microneedling than XERF.

Another 35-year-old may have beautiful skin texture but be genetically predisposed to early lower-face laxity. XERF might make more sense.

Someone else may have significant sun damage and pigmentation, in which case none of these treatments would necessarily be my first priority. A treatment such as Ultra LaseMD may be more appropriate for addressing those concerns.

This is why age alone doesn't determine treatment.

What About Your 40s?

This is often when patients begin noticing multiple components of aging at once. Maybe the skin doesn't feel as firm. There is likely now noticeable jowling. Pigmentation from years of sun exposure becomes more noticeable. Fine lines are beginning to remain even when the face isn't moving.

This is where combination treatment becomes especially valuable.

Rather than expecting one device to accomplish everything, we can address different layers of aging individually.

For example, a thoughtful plan might include:

  • XERF for deeper collagen support and firmness.

  • Ultra LaseMD for pigmentation, sun damage, tone, and texture.

  • Neuromodulators for excessive muscular movement and dynamic lines.

  • Biostimulatory treatments when additional collagen support or structural restoration is appropriate.

  • Medical-grade skincare and daily SPF to protect the investment we're making in the skin.

That's very different from simply saying, "You need filler."

And What About Your 50s and Beyond?

Collagen stimulation still matters. But expectations become especially important.

Energy-based treatments can improve skin quality and support collagen remodeling, but they cannot reproduce the results of surgery when significant tissue descent or laxity is present.

One of the most important things I can do as an aesthetic provider is tell a patient when a device isn't going to accomplish her goal.

Sometimes nonsurgical treatments are appropriate. Sometimes we can meaningfully improve skin quality while accepting that structural aging remains. And sometimes the most appropriate recommendation is a surgical consultation.

Good aesthetics isn't about selling every patient a treatment.

It's about knowing what our treatments can and cannot do.

Does More Downtime Mean Better Results?

No. This is another common misconception.

Patients sometimes assume that if a treatment hurts more, creates more swelling, or requires more recovery, it must be doing more. That's not how energy-based medicine works.

Downtime reflects the type and degree of tissue injury created by a treatment.

Effectiveness depends on whether we're creating the appropriate biological response in the appropriate tissue for the patient's concern.

A treatment can be aggressive and still be wrong for the problem.

A treatment can have virtually no downtime and still create meaningful tissue remodeling.

The goal isn't to create the most inflammation possible.

The goal is controlled, purposeful treatment.

Where Does Ultra LaseMD Fit Into All of This?

Ultra deserves its own category because it isn't a radiofrequency treatment. You can read more here about this device.

Ultra LaseMD uses a 1927 nm thulium laser.

While XERF allows us to focus more deeply on structural collagen remodeling, Ultra is particularly useful for concerns closer to the skin's surface, including:

  • Sun damage

  • Pigmentation

  • Uneven tone

  • Texture

  • Fine lines

  • Overall skin quality

This is one reason I don't think about devices as competitors. I think about them as tools.

A carpenter doesn't ask whether a hammer is better than a screwdriver. The answer depends on what needs to be built.

Sometimes the Best Treatment Is a Combination

The future of aesthetics isn't one miracle device.

It's intelligent treatment planning.

We can address muscular movement with neuromodulators.

We can restore structural support when appropriate.

We can stimulate collagen.

We can improve pigmentation and photodamage.

We can improve skin texture.

We can protect the skin with evidence-based skincare and sun protection.

And we can support the entire process with nutrition, exercise, sleep, and healthy lifestyle habits.

When these pieces work together, the result doesn't necessarily look “treated.”

Ideally, it simply looks like healthy, well-maintained skin.
